What this Policy Covers. This website, www.chemoorders.com (“Site”), is owned and operated by ChemoOrders.com, LLC. (“ChemoOrders.com,” “we” or “us”). Because your privacy is important to us, we have adopted the following policy regarding the use of your personal information that we receive from you through this Site. If you have any questions about this Privacy Policy, the practices of this Site, or your dealings with this Site, you can contact ChemoOrders.com at either address shown below. This Privacy Policy is made a part of the Service Agreement (available at www.chemoorders.com/Terms.aspx) that your employer entered into with ChemoOrders.com in order to use the Site, but governs the collection and use of any Personal Information (as defined below) you provide to us on the Site.

What is “Personal Information?” This Privacy Policy addresses our use of your “Personal Information” that we receive from you when you use our Site. For purposes of this policy, Personal Information means any information that identifies you, or can be used to identify, contact or locate you. Personal Information also includes information that, by itself does not identify you, but when combined with other information we have, can be used to personally identify, contact or locate you, for example, your name, address, phone number, fax number, email address, financial profiles, social security number and credit card information. ChemoOrders.com does not request social security number, credit card or financial information on its website at this time.

Personal Information also includes information we receive from you regarding another person that identifies that person, or can be used to identify, contact or locate that person, including by combining that information with other information we already have. ChemoOrders.com does not request, and affirmatively requests that you do not provide, personal information regarding any of your medical practice’s patients.

We obtain some Personal Information directly from you and other Personal Information may be collected automatically from you during your use of our Site. We also may already have information about you from various sources such as from your previous visits to our Site or from other third parties with whom we have joint marketing or similar arrangements. Our use of this information is discussed below.

Who Owns the Data that is Provided to or otherwise Collected or Created by the Site and/or the Engine (as defined in the Service Agreement)? ChemoOrders.com provides the Site free of charge, or for a nominal fee, in order to assist medical practices in developing chemotherapy orders for their patients. Useful health and research information can be developed by combining information that you provide during Site visits with those of other Site visitors. ChemoOrders.com owns all such information and may use such information for any purposes whatsoever except as expressly provided otherwise in this Privacy Policy. This Privacy Policy governs only our use of your “Personal Information” and does not otherwise limit our ownership, control or use of any other information you provide to us or the use of your Personal Information as allowed in this Privacy Policy.

How do We Collect and Use Your Personal Information? Some pages on our Site may contain forms that require users to provide contact information (such as name and e-mail address), demographic information (such as ZIP code), and other similar information. This information is needed to identify you to our Site and to provide you with information that you have requested. These forms and the information they request may change from time to time. Again, please do no provide any such information with respect to a patient – only with respect to yourself (if requested) and your employer.

We may use your IP address (see Glossary below) to help diagnose problems with our server and to administer our Site. The Site may also log IP addresses for systems administration purposes to track a user's session. This tracking also helps us evaluate which parts of our Site users are visiting so that we may improve our Site to better serve you. If you log onto your account with us (as opposed to visiting our Site without logging on) your IP address is linked to personally identifiable information, but only while your logon session is active. This linkage is necessary at that time for security reasons.

Our Site may use Cookies (see Glossary below) to keep track of your login session. Once you log out, your session is terminated and the cookie is removed. If your browser is set to reject cookies, you will not be allowed to login to our Site until you change the setting to accept the cookie.

We may use your Personal Information to provide you or your employer with information about ChemoOrders.com’s products and services and to evaluate your use of our Site so that we can improve our Site’s design and content. We may disclose information derived from you in an aggregate, statistical form (along with information about our other Site visitors) that does not identify you or your employer personally for research purposes and to companies who are interested in advertising on, marketing through or linking with our Site (“Advertising Partners”). We will not disclose your Personal Information to our Advertising Partners in a manner that would allow them to contact you directly unless you “opt in” to our use of your information in this manner. Security. Because any personally identifiable information you submit to ChemoOrders.com online is purely voluntary and should not be of a particularly sensitive nature, we employ our standard security measures and do not use special encryption methods at this time.

You can “opt out” of receiving e-mail, direct mail and other direct communications from us regarding our products and services (other than communications regarding information, products or services that you directly requested we contact you about). You may opt out by sending an e-mail to communications@chemoorders.com with “remove” in the subject line and request that we do not use whatever Personal Information we have on file pertaining to you in any e-mail, direct mail or other direct solicitations regarding ours or our Advertising Partners’ products and services. Please note that even if you opt out, you may still receive banner advertisements and other messages regarding our Advertising Partners’ products and services that are on our Site.

How can you Opt Out of Receiving Marketing Materials from our Advertising Partners? You can “opt in” to receiving e-mail, direct mail and other direct communications from us or our Advertising Partners regarding their products and services. How Can I Correct or Delete Any Personal Information You may have About Me? This is ChemoOrders.com’s sole obligation and your sole remedy for ChemoOrders.com’s breach of this Privacy Policy. Cookies

Cookies are messages that a web server sends to an Internet browser software program. The browser, in turn, stores the message on the personal computer hosting the browser. Temporary cookies only last as long as the browser is running. Persistent cookies are stored on the computer’s hard disk. Sites often use cookies to identify repeat visitors and offer them personalized service. At present, ChemoOrders.com only uses temporary cookies, if any.

IP Address

When you are connected to the Internet, your computer has a unique Internet ID called an IP address. Most people who connect through a dial-up service, such as America Online, get a different IP address each time they login. Others might have a permanently assigned IP address, called a static IP address. It is difficult or impossible for a website to collect Personal Information about you (i.e., your name, e-mail address, etc.) from your IP address alone, though a static IP address makes this easier. We do not at this time collect Personal Information in this manner, but we reserve the right to do so in the future.
